Hypothyroidism is a clinical condition caused by a deficiency of thyroid hormones, usually due to a thyroid gland disorder (primary hypothyroidism). The most common cause worldwide is iodine deficiency and chronic autoimmune thyroiditis in iodine- sufficient areas. Clinical manifestations are not specific, so diagnosis is based on laboratory tests and serum TSH concentration is the best screening test. The treatment of choice is levothyroxine individualizing initial dose according to the severity of hypothyroidism, age and history of ischemic heart disease. The treatment of subclinical hypothyroidism is controversial; it should be considered when TSH is greater than 10 mIU/ml, there are hypothyroidism clinical manifestations, goiter or positive thyroid antibodies, as well as in children population and pregnant and planning pregnancy women. Although overt hypothyroidism is associated with obstetric and neonatal complications, there is not sufficient evidence to recommend universal screening in pregnant women.
