Síndrome de Good, diarrea crónica y desnutrición

Zusammenfassung

Good syndrome is a rare entity that associates the presence of a thymoma with humoral immunodeficiency. Its clinical presentation is featured by recurrent infections. The most common ones are sinopulmonary infections accompanied by chronic diarrhea that can lead to significant malnutrition with a difficult therapeutic management as immunoglobulin replacement therapies do not necessarily improve the symptoms of diarrhea. Chronic diarrhea is associated with malabsorption, which can be related to the lesion of the mucosa, infection by gastrointestinal pathogens or bacterial overgrowth. We report the case of a 60-year-old man with Good Syndrome and recurrent episodes of diarrhea unresponsive to medical treatment, severely influencing the nutritional status of the patient. Eventually, in the face of his critical condition, we decided to use periodic cycles of antibiotics achieving an acceptable clinical response with significant improvement of the nutritional status and quality of life of the patient.
