Abstract

Introduction: Intervertebral disc degeneration (DIV) has been correlated with discogenic lumbar and cervical pain, is a serious health problem, as well as one of the leading causes of disability and represents an alarming health and socioeconomic problem. Platelet Rich Plasma (PRP) is a safe and effective therapeutic approach to musculoskeletal degenerative conditions and seems a clinically promising method on degenerate and injured discs. Methods: In this work we present our experience in the treatment of discogenic pain and evaluate changes in pain and function in 24 patients with discogenic cervical and lumbar pain following after epidural, paravertebral and intradiscal administration of PRP. Results: 24 patients (14 men and 10 women) have been treated. The initial Vas was 5.9 ± 0.7; 2 months of 2.8, ± 0.89 (p = 0.000); 6 months 1.4 ± 0.90 (p = 0.000) and 12 months of 2.1 ± 0.799 (p = 0.000). The initial ODI index was 43.20 ± 11.56; 2 months of 25.05 ± 7,783, (p = 0.100) of 21 ± 6,740; 6 months (p = 0.016) and 19.1 ± 7,498 at 12 months (p = 0.000). Conclusion: Our results are encouraging at 2, 6 and 12 months, using strict categorical diagnostic criteria, for the use of epidural and intradiscal PRP as a treatment for discogenic cervical and lumbar pain as it significantly decreased physical pain and disability Randomized placebo-controlled trials are needed to further evaluate the effectiveness of this treatment.
